Functional Medicine: Self-Pay

FUNCTIONAL MEDICINE Services with Tracy, FNP, and Mica, PA-C, are billed at an hourly rate

• Initial New Patient Appointments are $350.00. Typically 75 to 90 minutes.
• Follow-Up Appointments are $225.00 for up to 45 minutes.
• Extended Follow-Up Appointments are $300.00 for up to 60 minutes.
• If an appointment lasts over the scheduled appointment time, we prorate those fees accordingly.
• ENabling COgnitive REpair (ENCORE program) appointments are always 60-90 minutes
  

Physical Therapy: Self-Pay

PHYSICAL THERAPY Services with Dr. Madelyn, including FSM, are billed at an hourly rate

• Initial Evaluation: $225 - 90 minutes
• Follow-up: $225 - 90 minutes
• Follow-up: $150 - 60 minutes
• 15-minute add-on: $60 -  15 minutes 
  

TELEHEALTH PHYSICAL THERAPY SERVICES.

• $150 per hour (Initial Evals are 90 minutes @ $225)
• $150 per hour If an appointment lasts over the scheduled appointment time, we prorate those fees accordingly
  

Holistic Nutrition & Wellness Coaching

• Initial Session with Raechel is 1 hour, $150
• Follow Ups are 45 mins, $90

Energy Healing

• Ninty-minute sessions with Cathy, $210

InBody 720 Body Composition Analysis

Tracy , FNP, Mica, PA-C, and Rachael, Holistic Wellness Coach all provide InBody appointments

• $75: Scan and Interpretation, 15 mins appt
• $50: Add on to any appointment

One-on-one sessions with Dr. Madelyn are 1 hour, with handouts and homework prior to your session.

• $150 per hour

*Appointments are self- pay only. We are not in-network with any insurance company. We can provide Superbills to you upon request. Some plans, more commonly PPO’s, may directly reimburse you for some of the appointment cost.

*Appointment costs do not include lab or treatment costs. Many lab fees are billable through your insurance. Please contact your insurance company to know the details of your lab coverage prior to your initial appointment so we know what your most affordable lab options are.
